McBride Orthopedic Hospital is opening up their offices for all athletes to get their 2020-2021 Sports physicals. Please read below for more information.
PLEASE READ INSTRUCTIONS CAREFULLY AND COMPLETE EACH STEP:
* Call 405.230.9270 to schedule an appointment time. ($25 Physical Appointment Fee)
* Download and print Physical Form here (). Bring completed form to your scheduled appointment.
* Physicals will be held at all McBride Orthopedic Hospital Clinic locations.
PLEASE NOTE: YOU MUST WEAR A MASK OR FACE COVERING TO ENTER THE BUILDING.
